Adult Community Integration

Community Integration (COI) operated by Health Resources of Arkansas, Inc. is nationally accredited by the Commission on Accreditation of Rehabilitative Facilities (CARF). COI provides professional services addressing the areas of social, leisure, education, community and work preparation in the belief that persons with serious mental illness can achieve stability and reach their maximum rehabilitation potential through the provision of quality, goal-oriented treatment. This is achieved through the use of daily and weekly goals that are part of the master treatment plan of each individual served.

Community Integration is provided at three locations within the service area; Pine Valley, located in Augusta (Woodruff County), Cedar Mountain at Rose Bud (Cleburne County) and Evergreen in Batesville (Independence County). Transportation is available within designated areas surrounding each location.

Who Is Eligible For Services?

Adults, eighteen years of age and older with chronic psychiatric disabilities living in the service area and who meet the following criteria:

• DSM IV Classification or physician's certification for a chronic mental illness
• General medical condition does not preclude/restrict ability to participate in treatment activities, as verified by the medical data sheet and accompanying statement concerning physical health
• Individuals assessed as having potential to benefit from the program
• Individuals who are capable of performing activities of daily living with supervision and minimal assistance
• Individuals who are capable of functioning at a level which does not require one-to-one assistance
• Individual is assessed as appropriate by staff psychiatrist, referred by HRA Clinician, and packet complete
• Client or guardian must consent to treatment and should be willing to comply with the recommended treatment program
• Client referral packet indicates a pay source

Adults Who Are Not Appropriate for Community Integration

Community Integration is very goal-oriented and persons with the following characteristics should not be considered for services:

• Individuals that present convincing evidence of potential danger to self or others (suicidal ideation or attempt homicidal ideation) or violence toward others which requires a more restrictive environment
• Impairment solely caused by: a) epilepsy; b) mental retardation; or c) substance abuse/dependence. Persons should have a minimum IQ of 70, but exceptions may be made if the referring Clinician assess for need ability to function and ability to profit from services
• Severe dysfunctional patterns/behavioral processes which require a more restrictive treatment modality. (Persons that require inpatient crisis stabilization and/or inpatient diagnostic evaluation to determine treatment needs)
• Impaired reality testing is sufficient to prohibit participation in COI and who require a more intensive and/or restrictive treatment program

Hours and Days of Operation

Monday through Friday 8:30 am to 3:30 pm.  Days and hours of operation are reviewed biannually through the Client Advisory Council and based on feedback, adjustments are made to the schedule.

Services Provided

The COI programs provide goal-oriented, individualized support for persons served through the following service elements:

• Individual therapy is provided within the program as requested. Other services are provided through the outpatient program if needed or requested.
• Psychiatric/medical consultation, pharmacological and crisis intervention services are provided with Health Resources of Arkansas.
• Group therapy activities to enhance the understanding of psychiatric disabilities and the independence of those served.

• Grooming
• Stress management
• Medication Management
• ExerCOIe
• Academics and GED
• Community awareness
• Nutrition and meal planning
• Leisure activities

• Work environment activities are provided to develop work skills which may be transferred into the community. However, specific job placement may not be possible, although assistance with job resources and the job seeking process is within the capabilities of the program.
• Income management and entitlement issues are addressed through case management assistance in obtaining resources from appropriate agencies.
• Health maintenance, dental services, and pharmacy services are provided through community resources.
• Transportation is provided to and from COI. Transportation for all outings and activities is also provided.
• Family members and/or significant others are encouraged to participate in treatment and educational opportunities with the consent of the person served.
• Where applicable, COI staff will assist persons served to link with advocacy/self-help groups such as AA, ALANON/ALATEEN, Alliance for the Mentally Ill, etc.
